Personalised Health Targets
Join an annual plan and let medical professionals tailor monitoring standards for your loved one.
In-home Nurse Assessment
A registered nurse visits to assess health, build a personalised medication management system (timing, dosage reminders, interaction screening, regular reviews), and check the home environment (lighting, ventilation, anti-slip, furniture layout, emergency equipment) with improvement suggestions.
Personalised Targets
Set personalised blood pressure, glucose and SpO2 targets based on age, medical history and clinical advice. Unlike generic apps or the standard AngelCare version that use default values, upgraded ranges are not set by family members — they're tailored by medical professionals to your loved one's actual condition.
Data Sharing (coming soon)
Health indicators (e.g. red/amber/green alert ranges for blood pressure and glucose) are not set by family members — they involve clinical judgement. AngelCare entrusts this to our professional medical team or your attending doctor, who can remotely monitor health trends and fine-tune targets.
How It Works
Step 1: Contact us
Contact us via the website or WhatsApp. Our team learns about your loved one's condition, explains services and pricing, and confirms the appointment.
Step 2: In-home assessment
A registered nurse visits for home safety assessment, medication screening and a full health assessment (ICP), sets up AngelCare's core features on the spot, then provides personalised care recommendations.
Step 3: Personalised plan
Based on the assessment, we build a personalised care plan — nurse assessment, custom vital sign ranges and medical group consultation can be used alone or combined.
Step 4: Ongoing support
Vital signs are monitored regularly and your doctor can review data remotely; contact us anytime to adjust the care plan.
